How to fill it in

What it is for

The production insurance checklist confirms that cover for people, third parties, equipment and vehicles spans the whole production period with no gaps. The production manager uses it as a question list for the insurer or broker, and it is where everyone looks to see who does what after an incident.

When to use it

Skim the cover types when you build the budget, fill it in item by item at the quote and purchase stage two to three weeks before the shoot, and reopen it whenever days, risky scenes or locations are added.

Field by field

  • Coverage period (incl. prep & wrap-out)

    What
The full period cover must span
    How
From the first gear pickup or set build to the last return and site restoration — not just shooting days.
    Example
    2026-10-20 – 2027-01-27
  • Insurance contact

    What
Who owns insurance in the production office, and the insurer-side contact
    How
List both so anyone on set can call straight away.
    Example
    Hana Kim, PM, 010-0000-0000 · ○○ Brokers 02-000-0000
  • Incident reporting chain

    What
Who gets told, in what order
    How
From the first person on the scene to the insurer, with arrows. Put the same chain on the call sheet.
    Example
Set → production office → PM → broker
  • Area

    What
Item group (people, third-party liability, equipment, documents)
    How
Already filled in. Insert extra items for your project inside the right group.
    Example
    Third-party liability
  • Check item

    What
A risk to confirm is covered
    How
Keep the preset wording. If an item does not apply, do not delete it — write “n/a” and why in Notes.
    Example
Picture and unit vehicles — permitted drivers, own-damage cover, gear in transit
  • Basis / reference

    What
    Related law or guidance
    How
The preset references are summaries of Korean rules. Confirm eligibility and terms on the policy, the official text and with the relevant authority — outside Korea, use local requirements.
    Example
    Employment Insurance Act Art. 77-2
  • Done

    What
    Confirmed on the policy
    How
Tick ■ only when confirmed on the policy or certificate, not just the quote.
    Example
    ■
  • Owner

    What
    Who checks it
    How
The person who actually deals with the other side — equipment lead for rentals, location manager for location certificates.
    Example
    Geonyoung Lee, PA
  • Due

    What
When the check must be done
    How
Before the day the cover is first needed, such as gear pickup or location access.
    Example
    2026-10-24
  • Notes

    What
    Outcome and what is left
    How
Exclusions and deductibles the set needs to know. Copy amounts and limits exactly as on the policy.
    Example
    Awaiting broker reply

Tips

  • Send this sheet to the insurer or broker and ask for “covered / excluded / needs endorsement” per item, then copy the answers into Notes.
  • Flag risky scenes (action, height, water, SFX) with their shooting days as soon as the schedule exists so cover can be checked early.
  • Location owners often ask for a certificate of insurance — ask at the agreement stage and have copies ready.
  • Keep policy and certificate copies in the production drive and the unit vehicle, and put the insurer contact on every call sheet.
  • Premiums, limits and mandatory cover follow the contract and applicable law; copy them onto this sheet exactly as the policy states.

Common mistakes

  • Insuring shooting days only, so accidents on build or wrap-out days are not covered.
  • Trusting that the rental house “has insurance” without checking the deductible and theft cover.
  • Adding shooting days but forgetting to extend the policy, leaving the last week uninsured.
  • Trying to record an incident only afterwards and missing photos and witnesses, which makes claims hard.

FAQ

Why do locations ask for a certificate of insurance?

Owners usually want proof of liability cover for injury or damage to third parties during the shoot. Check the limit and that the location and dates are listed.
